PATIENT PORTAL is now available!

Lake Street Family Physicians is pleased to offer a secure communication service called Patient Portal. The Patient Portal is provided as a convenient way to request non-emergent appointments, prescription refills, lab results and communicate with our office via a secure email service.

Due to the secure nature of the Patient Portal, requests for access must be made in person. Ask for more information and to sign up during your next visit to our office.

Located on downtown Oak Park on Lake St. in a 6-story white professional building near the Lake movie theatre. We are also conveniently located one block away from the Oak Park stop of METRA and the Harlem/Lake stop of the Green Line of the CTA. We are on the Pace Bus Line routes as well.

PARKING: Village of Oak Park parking garage located on Forest Ave, just north of Lake Street. The first hour is free.
